Insert Table of Contents into document

Currently, a Table of Contents is added automatically to the top of the page when you have at least 4 headers. To force a table of contents to appear anywhere in the page (even if you don't have 4 headers), use __TOC__ in the required area. To remove the table of contents completely, use __NOTOC__.
